Preoperative Diagnosis: Undesirable Nasal Appearance desiring Westernization; s/p rhinoplasty X 2 with dorsal implant, microgenia
Procedure: Tertiary Revision westernization rhinoplasty with alar base reduction and goretex/conchal cartilage implant, removal of silicone dorsal implant, septoplasty, right ear conchal graft, chin implant and platelet gel application.
Findings: After removal of silicone implant, tip and dorsum needed reconstruction. For tip, a columellar strut, shield graft and plumping grafts used. For dorsum, patient chose maximal dorsal augmentation with a combination of stacked goretex and bruised conchal cartilage. Weir incisions were performed for alar base reduction.
Comment: Her post op photos reveal a healing improved nose.
