Patient with a history of a cleft lip and nose and following a rhinoplasty, now complains of the appearance of her nose and the recessed space between her nose and her lip.
Findings at the time of surgery were over-resected lower lateral cartilage (left greater than right) with minimal medial crural , saddle nose deformity, bossa formation, external valve collapse, superior caudal septum deviated to left with no tip support collapsing to left.
Procedure: Revision Rhinoplasty with ear cartilage grafting for reconstruction of lower dorsum (saddle nose deformity), complete tip reconstruction with lateral crural struts and shield graft, septal batten/strut graft and a Maxillary Goretex implant to help augment the space between her lip and nose.
Comment: Post-operative view after 2 months revealing a softer, yet natural nose.
