PREOPERATIVE DIAGNOSIS:
Residual external nasal deformity, deviated septum, nasal obstruction, s/p rhinoplasty X 2, and hypertrophic turbinates.
PROCEDURE:
Revision Tertiary Ethnic Open rhinoplasty, septoplasty and bilateral turbinoplasty.
FINDINGS:
Previous stiar-step incision; 3+ scar encompassing nasal tip and caudal middle vault; conchal cartilage graft as columellar strut and right alar graft identified and removed; 3+ thick nasal skin; mild cartilaginous hump with low radix and bony dorsum; narrow left lateral nasal wall; asymmetric ala with right wider than left; bilateral weir incision wound separation; cephalically oriented and weak right lower lateral cartilage (shorter than the left LLC) and septum deviated to left with spur.
COMMENT:
This patient wished to reduce a hump and achieve better alar symmetry. Post-operative photos reveal an improved nose and more aesthetically pleasing profile.
