PREOPERATIVE DIAGNOSIS:
- Cleft nose deformity, left synechia, maxillary hypoplasia, oral antral fistula, nasal obstruction, deviated septum, bilateral turbinate hypertrophy.
PROCEDURE:
Revision Open Secondary Cleft Septorhinoplasty, bilateral turbinate reduction, right auricular composite graft, maxillary implant, rib harvest.
FINDINGS:
Poor projection, pyriform/maxillary retrusion (left greater than right), concave left lower lateral cartilage, deficient left vestibular tissue, composite graft 7x19mm placed on left rim, minimal remnant left medial crura. Silicone left rim graft, 2 x 10mm oriented vertical.
COMMENT:
This patient wished to revise a previous cleft septorhinoplasty (different surgeon) and improve breathing. Post-operative photos reveal a softer profile and improved nasal appearance. Improved breathing has been reported. Patient continues to heal.
