PREOPERATIVE DIAGNOSIS:
Nasal dysmorphia status post rhinoplasty
POSTOPERATIVE DIAGNOSIS:
Same and Septal Deviation. Bilateral medialized turbinates and nasal obstruction secondary to medialized turbinates, left external nasal valve weakness, and deviated septum.
PROCEDURE:
External Secondary Revision Rhinoplasty, Revision Septoplasty, Bilateral Turbinate Outfracture, Right Composite Ear Cartilage Harvest.
- Repair of left nasal vestibular stenosis secondary to left external valve weakness secondary to left soft tissue triangle deficiency.
- Complex revision septal reconstruction, submucous resection with major partial vomerectomy for correction of deviated nasal septum.
- Outfracture of left inferior turbinate.
- Outfracture of right inferior turbinate.
- Right composite ear cartilage harvest.
- Removal of nasal implant.
FINDINGS:
Short over-rotated, underprojected nose, vestibular and skin deficit in left soft tissue triangle, twisted silicone dorsal implant, short caudal septum. Septal bony deviation to left, bilateral medialized inferior turbinates.
COMMENT:
This patient wished to revise a previous rhinoplasty which left her with unsatisfactory results (different surgeon). Post-operative photos reveal a more natural profile. Breathing has improved as well. Patient still swollen in post-operative photos.
