PREOPERATIVE DIAGNOSIS:
- Nasal obstruction secondary to left internal valve collapse following rhinoplasty.
PROCEDURE:
- Repair of nasal vestibular stenosis with reconstruction of left internal valve collapse with placement of a left spreader graft.
- Graft, left ear cartilage
- Fracture of nasal turbinates
- Lysis of intranasal synechiae
FINDINGS:
Right posterior septal spur (septoplasty not performed secondary to risk of septal perforation and patient not complaining of right airway obstruction), over-resected left greater than right short lower lateral cartilages (LLC) (approx. 5 mm strips), narrow left middle vault, cephalically-oriented LLC; large, infractured inferior turbinates, asymmetric columella; synechaie left intranasal inferior turbinate to septum; 3+ polly beak scar; under-projected tip; ridge left nasal bone, right greater than left retracted ala.
COMMENT:
This patient wished to correct the result of a previous unsuccessful rhinoplasty (different surgeon) and improve the appearance of their nose. Post-operative photos reveal an improved profile and a softer nasal appearance.
