PREOPERATIVE DIAGNOSIS:
- Nasal obstruction secondary to deviated septum, turbinate hypertrophy and external/internal valve collapse, s/p rhinoplasty with poor tip support and possible Binder’s syndrome (maxillary hypoplasia).
- Turbinate Hypertrophy right and left.
- Septal Deviation.
PROCEDURE:
- Septal reconstruction for correction of deviated nasal septum.
- Submucous resection and partial resection of the left turbinate and submucous resection and partial resection of the right turbinate
- Repair of nasal vestibular stenosis with reconstruction of external valve collapse with placement of bilateral alar rim grafts.
- Repair of nasal vestibular stenosis with reconstruction of subnasale deficiency with placement of plumping grafts.
- Repair of nasal vestibular stenosis with reconstruction of left internal valve collapse with placement of left double spreader grafts.
- Repair of nasal vestibular stenosis with reconstruction of tip support with placement of a columellar strut.
- Graft, right ear cartilage.
- Repair of open roof deformity with bilateral osteotomies.
- Secondary Open Rhinoplasty, platelet gel application
FINDINGS:
Left septal spur along floor of nose and caudal deviation to right; hypertrophic turbinates, 3+ narrow left middle vault; ptotic tip due to poor maxillary support and weak rims.
COMMENT:
This patient was having trouble breathing and wanted to improve the appearance of her nose. Post-operative photos reveal a softer profile and patient has reported improved breathing as well.
