PREOPERATIVE DIAGNOSIS:
- Nasal obstruction secondary to deviated septum, turbinate hypertrophy, external valve collapse, internal valve collapse and open roof deformity (s/p nasal trauma) causing nasal inlet obstruction.
- urbinate Hypertrophy.
- Septal Deviation.
PROCEDURE:
- Submucous resection and partial resection of left turbinate and submucous resection and partial resection of right turbinate.
- Complex septoplasty for correction of deviated nasal septum.
- Repair of nasal vestibular stenosis secondary to bilateral external valve collapse with bilateral subcartilaginous lateral crural strut grafts.
- Repair of nasal vestibular stenosis secondary to open roof deformity with bilateral osteotomies.
- Repair of nasal vestibular stenosis secondary to internal valve collapse with placement of bilateral spreader grafts.
- Open Septorhinoplasty.
FINDINGS: Collapsed external valve (concave LLC) left greater than right impinging into airway, Large septal spur to left with bony deviation, narrow middle vault, deviated nasal bones, right alar retraction, 3+ thick skin, poorly projected dorsum, weak lower lateral cartilages (LLC), and nose convex to left.
COMMENT: This patient wished to improve the appearance of her nose and fix a deviated septum. 7-month photos reveal an improved profile, and patient has reported improved breathing as well. Patient continues to heal.
