PREOPERATIVE DIAGNOSIS: Unacceptable nasal appearance, nasal obstruction secondary to hypertrophic turbinates, bilateral compensatory hypertrophic turbinates, septal deviation
PROCEDURE: Open rhinoplasty, platelet gel application, complex septal reconstruction, submucous resection with major partial vomerectomy for correction of deviated nasal septum, submucous resection and partial resection of right inferior turbinate, submucous resection and partial resection of left inferior turbinate.
FINDINGS: Anterior caudal septum deviated to right, left alar base wider than right, extreme vertically-oriented , weak, concave lower lateral crura, left large septal spur along floor of nose.
COMMENT: This patient had breathing issues due to a deviated septum. She was unhappy with the shape of her nose, and desired a change in the appearance. 3 year photos reveal a softer nose and improvements in breathing have been reported
