PREOPERATIVE DIAGNOSIS:
- Nasal obstruction secondary to external/internal valve collapse, deviated septum, following rhinoplasty, hypertrophic turbinates.
PROCEDURE:
- Repair of nasal vestibular stenosis with reconstruction of external valve collapse with placement of bilateral lateral crural strut grafts.
- Repair of nasal vestibular stenosis with reconstruction of tip support with placement of columellar strut.
- Repair of nasal vestibular stenosis with reconstruction of internal valve collapse with placement of left spreader graft.
- Graft, right ear cartilage.
- Right deep temporalis fascia harvest via brow lift.
- Septoplasty.
- Bilateral Turbinoplasty.
- Secondary Rhinoplasty, platelet gel application.
FINDINGS: Native: 5 mm over-resected short LLC (lower lateral cartilage) on right; left LLC 7 mm wide and concave; Ear cartilage used for shield graft and rim grafts. Weak rims. Narrow left middle vault and lateral nasal wall, step-off at rhinion with over-resected cartilaginous dorsum; residual bony hump; right greater than left retracted ala; left greater than right external valve collapse, poor tip projection. Caudal septum deviated to right with posterior deviation to left.
COMMENT: This patient was having problems with her breathing and desired an improvement in her nose. 7 month post-operative photos reveal a softer nasal appearance, and patient has experienced improvements in her breathing. Nose is still swollen in these photos.
