PREOPERATIVE DIAGNOSIS:
- Nasal obstruction secondary to deviated septum, turbinate hypertrophy, internal/external valve collapse and deviated nasal bones causing nasal inlet obstruction.
- Turbinate Hypertrophy.
- Septal Deviation.
PROCEDURE:
- Submucous resection and partial resection of left turbinate and submucous resection and partial resection of right turbinate.
- Complex septoplasty for correction of deviated nasal septum.
- Repair of nasal vestibular stenosis secondary to bilateal external valve collapse with bilateral subcartilaginous lateral crural strut grafts.
- Open repair of nasal fracture with osteotomies.
- Repair of nasal vestibular stenosis secondary to internal valve collapse and to support severely deviated septum with placement of bilateral spreader grafts.
- Repair of nasal vestibular stenosis secondary to left external valve collapse with left rim collapse with placement of left rim graft.
- Repair of nasal vestibular stenosis secondary to tip ptosis with placement a columellar strut.
Open Rhinoplasty, Platelet gel application.
ANESTHESIA: TIVA
FINDINGS:Collapsed left external valve greater than right impinging into airway, Large septal spur to left with bony deviation on left and caudal septum deviated to left, narrow left greater than right middle vault, splayed left nasal bone, right nasal bone with vertical/narrowed component. Native lower lateral cartilages: 8 mm.
