Female patient with nasal obstruction secondary to external/internal valve collapse following rhinoplasty X 2 and hypertrophic turbinates.
Findings: Narrow bilateral external valve with right worse than left and divided dome; 3 + scar debulked from tip, 3+ inverted V with left greater than right, composite grafts (5 mm X 15 mm); open roof deformity; significant alar retraction with Right alar retraction more than left, over-rotated, short nose.
Procedure: Tertiary Revision rhinoplasty with bilateral composite grafts, conchal cartilage harvest, deep temporalis fascia grafts, spreader grafts, radix/upper dorsal onlay graft, columellar strut, lateral crural strut grafts and infratip lobular graft
Comment: Her postoperative nasal appearance reveals an improved and yet functional nose. Her airway is improved.
