Description: This patient wanted a more masculine jawline. Dr. Paul Nassif performed a chin implant, necklift, and submental liposuction in his Beverly Hills office. The patient also had a rhinoplasty. The post-operative photos show a nice improvement and better balance between the nose and chin.
Preoperative Diagnosis: Nasal obstruction secondary to external/internal valve collapse, deviated septum, hypertrophic turbinates.Submental & jowl lipodystrophy, platysmal banding, and microgenia.
Procedure:
- Repair of nasal vestibular stenosis with reconstruction of external valve collapse with placement of bilateral lateral crural strut grafts.
- Repair of nasal vestibular stenosis with reconstruction of internal valve collapse with placement of bilateral spreader grafts.
- Repair of nasal vestibular stenosis with reconstruction of left external valve collapse with placement of a left rim graft.
- Septoplasty.
- Bilateral Turbinoplasty.
- Repair of nasal vestibular stenosis with repositioning of lower lateral cartilage secondary to vertically-oriented positioning
- Repair of nasal vestibular stenosis with tip weakness with placement of columellar strut.
- Repair of nasal vestibular stenosis with twisted bones with bilateral osteotomies.
- Open Primary Septorhinoplasty, platelet gel application.
- Submental and jowl liposuction and platysmaplasty, chin implant, platelet gel application.
Findings: Left > right external valve weakness, rim weakness, 3+ middle vault weakness/narrowing; septum with C-shaped deformity with anterior septum convex to right; 3+ spur at base of left airway on septum; septal-crural ligament lysed to deproject; twisted nasal bones; vertically-oriented LLC (lower lateral cartilages) with left LLC cephalad to Right LLC; left wide dome; asymmetric dome, left LLC concave. Boxy, splayed and bifid tip.
